This form is used to request access to records under the Promotion of Access to Information Act (PAIA). It includes sections for your personal details, the record description, access preferences (like printed copy, email, or cloud share), and more.

To request access to records under the Promotion of Access to Information Act (PAIA), please complete Form 2 and submit it to our Information Officer at paia@tlo.co.za. Include any applicable fees as outlined in our PAIA Manual. We will respond within 30 days as required by law.
